A Care Guardian Nurse Coordinator is a registered nurse who provides care coordination, medical and health education, and support services to assist members in the transition to health care and supports member empowerment through health education and mentoring. The Care Coordinator ensures continuity and quality of care for members with multiple complex conditions and chronic illnesses who have self-management goals. The individual in this role coordinates care with a three-goal approach: improve the member experience, improve population health and reduce healthcare costs.

Basic tasks
- Served as team leader for Care Signal functions, workflows and communications, development and maintenance of communications workflows, and promotion of health programs and services.
- Use information systems and decision support, lead a panel of members to proactively contact, educate and monitor members based on disease, risk and self-management status, and family and community needs. Provide resources to support members/families in self-management of health care goals, assessment of learning readiness, and validation of learning outcomes.
- Engage members and/or families in raising caregiving questions and concerns, and recognize the member's individual expertise through guidance and counseling in accordance with program guidelines.
- Facilitate advance planning, education and empowerment of members, and assessment of member barriers to support the most effective and efficient communication between members, resources and service providers.
- Collaborate with multidisciplinary teams and healthcare team members, both internal and external to the organization, to improve member disease management through effective use and monitoring of healthcare resources.
- Assist member with transition to hospital or emergency physician and ensure hospitalization, hospital and/or specialist follow-up, ensure member understands medications and treatments, and link member to appropriate resources for additional support. Interactions, findings, and continuity of care activities are documented appropriately and timely to ensure optimal member navigation reporting on quality improvement efforts.
- Participate in individual level performance evaluations with each member/client while participating in agency-wide evaluations to ensure and improve the overall quality of service provided.
- Collaborate with internal and external departments to improve member outcomes by reviewing member care gaps, educating members on the importance of preventive care and ongoing needs assessments, staying current on quality initiatives and Texas Health Aetna quality priorities.
