The Care Guardian Care Management Associate (CG CMA) facilitates the intake and screening processes for Care Guardian's remote monitoring and member acquisition programs through targeted communication. The Care Guardian program ensures continuity and quality of care for members who meet certain criteria with the goal of improving members' ability to manage their own health. The incumbent in this position will provide navigation of health services according to the three goals of public health: improving the member experience, improving outcomes (closing gaps in care), and reducing overall health care costs.
Basic tasks
- Act as a team coordinator, assisting with reporting, monitoring and workflows and communication, developing and maintaining communication workflows, and promoting wellness programs and services.
- Supports the identification and appointment of Care Guardian team members. Manages the referral mailbox and facilitates the exchange of information when necessary.
- Determines eligibility and identifies critical information needed to support initial clinical team engagement and ongoing management of members.
- Begin contacting patients to participate in the team using motivational interviewing techniques and explain the value of the program to patients
- Promotes communication both internally and externally to improve the effectiveness of the CG team, including communication with provider/office staff and other care management resources.
- Use existing electronic information systems (system or provider) based on authorized access to those systems.
- Provides support to team members by answering phone calls, taking messages, researching information and assisting with problem solving.
- Facilitates on-demand team performance reporting.
- Participates in efforts to optimize member engagement and program communications.
- Use information systems and decision support, proactively contact and inform members of care gaps, and identify potential public self-sufficiency status members and family and community needs.
- Other duties/projects as assigned
